יְהִי רָצוֹן מִלְּפָנֶיךָ
ה׳ אֱלֹהֵינוּ וֵאלֹהֵי אֲבוֹתֵינוּ
שֶׁהַחִסּוּן שֶׁאָנוּ עוֹמְדִים לְקַבֵּל
יָבִיא רְפוּאָה, בְּרָכָה וְהַצָּלָה
וְיָגֵן עֲלֵינוּ מִיִּסּוּרִים וּמִמַּחֲלוֹת קָשׁוֹת.
May it be your will
YHVH (Hashem) our god and god of our forefathers,
that the vaccinations that we are about to receive
will bring health, blessing and redemption,
and protect us from suffering and terrible diseases.

שֶׁהַחִסּוּן יַשְׁפִּיעַ בִּמְהִירוּת
תּוֹךְ מִעוּט כְּאֵבִים וּלְלֹא תּוֹפְעוֹת לְוַאי,
וְשֶׁבִּזְכוּת מַתַּן חִסּוּנִים אֵלּוּ
תָּגֵן עַל תִּינוֹקוֹת בְּנֵי יוֹמָם,
עַל קְשִׁישִׁים וְעַל חוֹלִים
שֶׁאֵינָם יְכוֹלִים לְהִתְחַסֵּן.
May this vaccination work quickly
with minimal pain and side effects.
Through this vaccine, may we merit blessing
and protection for infants,
the elderly, and the sick
who cannot be vaccinated.[1]Vaccinations safeguard populations with weak or compromised immune systems through herd immunity. Without a large enough segment of the community vaccinated, dangerous diseases will spread and vulnerable populations imperiled. –Aharon Varady

וִיהִי רָצוֹן
שֶׁיִּזְכּוּ הָאֲחָיוֹת, הָאַחִים, הָרוֹפְאוֹת וְהָרוֹפְאִים
הַמְּחַסְּנִים אוֹתָנוּ
לְחַיִּים אֲרֻכִּים וּשְׁלֵוִים
לְפַרְנָסָה טוֹבָה וּלְהַצְלָחָה
וְשֶׁזְּכוּת הַשְּׁמִירָה עַל בְּרִיאוּת הַצִּבּוּר
תַּעֲמֹד לָהֶם עַד סוֹף יְמֵיהֶם.
May it be your will
that the nurses and doctors
who provide our vaccines
enjoy long and peaceful life,
financial security and success.
May the merit of their preservation of our health
stand in their stead until the end of their days.

בִּזְכוּת קִיּוּם מִצְוַת
וְנִשְׁמַרְתֶּם מְאֹד לְנַפְשֹׁתֵיכֶם (דברים ד:טו חלק),
נִזְכֶּה לְגַדֵּל אֶת בָּנֵינוּ וּבְנוֹתֵינוּ
לְתוֹרָה, לְחֻפָּה וּלְמַעֲשִׂים טוֹבִים.
בָּרוּךְ שׁוֹמֵעַ תְּפִלָּה.
Through the merit of observing the commandment,
“you should guard yourself carefully,”[2]Deuteronomy 4:15 part. 
may we be privileged to raise children
to Torah, the marriage canopy, and good deeds.
Blessed is the One who hears prayer.

In memory of my brother Dr. Sholom Wacholder z”l, who died on Hoshana Raba 5776. As a biostatistician for the National Cancer Institute, he was instrumental in the development of the vaccine to prevent cancers caused by HPV.
