רִבּוֹנוֹ שֶׁל עוֹלָם, יְקִירָתֵנוּ (פלונית בת פלוני ופלונית) נִלְקְחָה לְבֵית עוֹלָמָהּ. כּוֹאֲבִים וְדוֹאֲבִים, אָנוּ מְבַכִּים אֶת הִסְתַּלְּקוּתָהּ.	Master of the Universe, our beloved [name of the departed], daughter of [father’s name and mother’s name] was taken to her eternal resting place. In pain and grief, we mourn her loss.
אָבִינוּ שֶׁבַּשָּׁמַיִם, בִּשְׁעַת אֲנִינוּת קָשָׁה זוֹ, כְּשֶׁמֵּתֵנוּ מֻטָּל לְפָנֵינוּ, מְבַקְּשִׁים אָנוּ לְהוֹסִיף חַיִּים, טוֹבָה וּבְרָכָה בָּעוֹלָם, עַל פִּי הַשְׁקָפָתָהּ שֶׁל הַמְּנוֹחָה. וְעַל כֵּן בָּאִים אָנוּ לִתְרֹם אֵיבָרִים מִגּוּפָהּ לְקַיֵּם מִצְוַת פִּקּוּחַ נֶפֶשׁ, לְהַצִּיל חַיִּים וּלְהַעֲנִיק רְפוּאָה לְחוֹלִים הַזְּקוּקִים לִישׁוּעָה, וְאוֹמֵר הַכָּתוּב: ”עוֹלָם חֶסֶד יִבָּנֶה“ (תהלים פט:ג). תַּעֲמֹד זְכוּת זוֹ לְעִלּוּי נִשְׁמַת הַנִּפְטֶרֶת.	Our Father in Heaven, in this difficult hour of acute mourning, when the deceased lies before us, we seek to give life to this world, and bring goodness and blessing into it. In harmony with the worldview of our beloved relative, we donate organs from her body to fulfill the mitsvah of piḳuach nefesh, to save lives and offer healing to the sick who are in dire need of salvation. As Scripture says: “The world is built through lovingkindness” (Psalms 89:3). May the merit of this deed endure, and accompany the ascension of the soul of the deceased.
וִיהִי רָצוֹן מִלְּפָנֶיךָ, מֶלֶךְ חַי וְקַיָּם, שֶׁהַשְׁתָּלַת הָאֵיבָרִים בְּגוּפָם שֶׁל הַנִּתְרָמִים תִּצְלַח, וְיִזְכּוּ הֵם לִרְפוּאָה שְׁלֵמָה בִּמְהֵרָה וּלְהַאֲרִיךְ יָמִים וְשָׁנִים בַּטּוֹב וּבַנְּעִימִים.	May it be Your Will, everlasting king, that the transplantation of the organs in the bodies of the recipients be successful. May they have a complete recovery soon, and may You prolong their days and years in good health, peace and prosperity.
בּוֹרֵא עוֹלָם, בּוֹחֵן כְּלָיוֹת וָלֵב, הַבֵּט נָא מִשָּׁמַיִם וּרְאֵה עֲמִידָתֵנוּ וְהִתְעַסְּקוּתֵנוּ בְּשָׁעָה זוֹ בְּהַצָּלַת נְפָשׁוֹת. רַחֵם עָלֵינוּ וְחַזְּקֵנוּ וְאַמְּצֵנו עַתָּה לְאַחַר אָבְדַן יְקִירָתֵנוּ. כִּי אֵל שׁוֹמֵעַ תְּפִלּוֹת וְתַחֲנוּנִים אַתָּה.	Creator of the world, examiner of the heart and mind, look down, please, from heaven, and see our efforts at this time in saving lives. Have compassion upon us, strengthen us, and help us in this hour following the loss of our beloved one. For You are God who hears prayers and supplications.

“A Prayer for Relatives to Recite Prior to the Donation of Organs of their Beloved (for a female)” by Dr. Yael Levine is the fourth of four prayers offered by the author in 2024 in the event of an organ transplant procedure. Dr. Levine writes, “תפילות אלו התחברו על ידי לפי פנייתו של הרב ד”ר שלמה ברודי אליי עבור ארגון “אימתי” שהוא עומד בראשו, ועלו שם לאתר במחצית שנת 2024. (These prayers were composed by myself at the request of Rabbi Dr. Shlomo Brody for the “Ematai” organization that he heads, and were uploaded to the website in mid-2024.)

Yael Levine

Dr. Yael Levine (Heb: יעל לוין) holds a Ph.D. from the Talmud department of Bar-Ilan University. She has published numerous studies focusing on issues related to women and Judaism. She has also composed many prayers and midrashim.

Initially founded in 2001 to educate about organ donation in Jewish communities, ɐ̤matai was relaunched in 2023 to enable Jewish families to navigate an increasingly complex healthcare journey, particularly in the dilemmas of aging and end-of-life treatment.
