An adaptation of the fourth berakhah of a festival musaf for Sigd. One technical note: as there is technically no Sigd musaf in the liturgical framework historically established within the halkhah of rabbinic Judaism, this berakhah does not end with shem u-malkhut in the ḥatimah.
Note: “The CAUSE” is used to translate the Divine Name YHVH, based on the philosophical idea of God as the Prime Mover and on the interpretation of the Name as a causative form of the copula – “causes to be.” “God” is used to translate the divine name Elohim. All divine referents (pronouns, epithets, names) are rendered in unicase.

יְהִי רָצוֹן מִלְּפָנֶֽיךָ יהוה אֱלֹהֵֽינוּ וֵאלֹהֵי אֲבוֹתֵֽינוּ מֶֽלֶךְ רַחֲמָן שֶׁתָּשׁוּב וּתְרַחֵם עָלֵֽינוּ וְעַל־מִקְדָּשְׁךָ בְּרַחֲמֶֽיךָ הָרַבִּים וְתִבְנֵֽהוּ מְהֵרָה וּתְגַדֵּל כְּבוֹדוֹ׃ |
May it be willed before You, Cause our God and God of our ancestors, compassionate Sovereign, that You return and have compassion on us and on Your Temple in great compassion, and speedily rebuild it and greaten its glory. |

But the addition for this Day of Prostration we will do as You wrote for us in Your Torah by the hands of Moshe Your servant, from Your holy mouth, as it is said: |

And Moshe commanded the people on that day, quote: “These will stand to bless the people on Mt. Gerizim upon your crossing the Jordan: Shim’on and Levi and Yehuda and Yissachar and Yosef and Binyamin. And these will stand for the curse on Mt. ‘Eval: Reuven and Gad and Asher and Zvulun, Dan and Naftali. And the levites will announce and say to each Israelite person aloud.” (Deuteronomy 27:11–14) |

Our God and God of our ancestors, compassionate Sovereign, have compassion on us; good one who does good, seek us, return to us in Your myriad compassions for the sake of the ancestors who did Your will; build Your house as it was and establish Your temple on its foundation that we may se its building and rejoice in its repair, and return the priests to their labor and the levites to their song and their hymn, and return Israel to their abode, and there we will ascend and be seen and bow down and prostrate before You on all the occasions of our times, as the matter is said in the words of Neḥemiah Your governor: |

And they arose on their pulpits and read from the book of the Torah of the Cause their God a quarter of the day, and for a quarter of the day they confessed and prostrated themselves to the Cause their God. And getting up on the levite ascent, Yéshua’ and Bani, Qadmiel, Shvanya, Buni, Sherevya, Bani, Knani, and they cried in a loud voice to the Cause their God. And the levites, Yéshua’ and Qadmiel, Bani, Ḥashavnya, Sherevya, Hodiya, Shvanya, Ptaḥya, said: “Arise! Bless the Cause your God from eternity to eternity, and may Your honored name be blessed and exalted over all blessing and praise!” (Nehemiah 9:2–5) |
וְהַשִּׂיאֵֽנוּ יהוה אֱלֹהֵֽינוּ אֶת־בִּרְכַּת מוֹעֲדֶֽיךָ לְחַיִּים וּלְשָׁלוֹם לְשִׂמְחָה וּלְשָׂשׂוֹן כַּאֲשֶׁר רָצִֽיתָ וְאָמַֽרְתָּ לְבָרְכֵֽנוּ׃ |
And bring us, Cause our God, Your seasons’s blessing, for life and for peace, for joy and for happiness, as You desired and said to bless us. |

Our God and God of our ancestors, may our remembrances and our accountings and the remembrance of our ancestors and the remembrance of the anointed son of David Your servant and the remembrance of Jerusalem Your holy city and the remembrance of all Your people the house of Israel ascend and come and reach and be seen and be desired and be heard and be accounted and be remembered before You for escape, for grace, for mercy and for compassion, for life and for peace on this Day of Prostration, the day of accepting our covenant. Remember us, Cause our God, on it for good, and account for us on it for blessing, and save us on it for life, and in a matter of salvation and compassion grace and have compassion on us and save is, for to You are our eyes, for a sovereign merciful and compassionate Deity are You. |
קַדְּשֵֽׁנוּ בְּמִצְוֺתֶֽיךָ וְתֵן חֶלְקֵֽנוּ בְּתוֹרָתֶֽךָ שַׂבְּעֵֽנוּ מִטּוּבֶֽךָ וְשַׂמְּחֵֽנוּ בִּישׁוּעָתֶֽךָ וְטַהֵר לִבֵּֽנוּ לְעׇבְדְּךָ בֶּאֱמֶת וּדְבָרְךָ אֱמֶת וְקַיָּם לָעַד׃ |
Sanctify us in Your commandments and grant us a portion in Your law, sate us with Your good and make us rejoice in Your salvation and purify our hearts to serve You in truth, and Your word is true and enduring forever. |
בָּרוּךְ אַתָּה מְקַדֵּשׁ יִשְׂרָאֵל וְהַזְּמַנִּים׃ |
Blessed are You, who sanctifies Israel and the times. |
