סַמִּי וּמֶרְקָחִי לְךָ – בֵּין טוֹב בֵין רָע, וּבֵין חָזָק וּבֵין רָפֶה.	My medicines are from you, whether good Or evil, whether strong or weak.
אַתָּה אֲשֶׁר תִּבְחַר וְלֹא אָנִי, עַל דַּעְתְּךָ הָרָע וְהַיָּפֶה.	It is you who shall choose, not I;^[1] S. D. Luzzatto says here, ‘”not I” instead of “not the physician” because Jehudah Halevi himself was the physician.' Of your knowledge is the evil and the fair.
לֹא עַל רְפוּאָתִי אֲנִי נִסְמָךְ – רַק אֶל רְפוּאָתְךָ אֲנִי צוֹפֶה.	Not upon my power of healing I rely; Only for your healing do I watch.

In Margaliyot Tovot (“Precious Pearls,” 1665), Yaaqov ben Yitsḥaq Tsahalon abridged Baḥya ibn Paquda’s Ḥovot ha-Levavot (“Duties of the Heart,” ca. 1080) and interspersed it with prayers including the following prayer for healers (Tefilat ha-Rof’im) which he recommended should be recited by physicians at least once every week.

(Job 33:21-22) But if he repents completely before you, you are pleased with him and you will cure him as it is written: “They sent Their word and healed them and delivered them from their graves” (Psalms 107:20). For you are the merciful and faithful healer, as it is written: “I have wounded and I heal” (Deuteronomy 32:39), in order to make known to all the denizens of the world that you are the healer and unto you is the power and the glory. From your established habitation you look down upon all the inhabitants of the earth. For their special benefit, have you created in your world many drugs by which to cure the sons of man and in your wisdom you have commanded them to cure one another by means of these medicines, as it is written: “They shall cause him to be thoroughly healed” (Exodus 21:19).
והואיל וחסדים חננת את לבי וכבוד והדר עטרתני וזכיתני לידע מעט מזער מחכמת הרפואה לכן לעשות רצונך אלהי חפצתי ומשום אל תמנע טוב מבעליו בהיות לאל ידך לעשות דברתי אני עם לבי להתעסק במלאכת הרפואה בשמך הקדוש ובעזרתך למען תצדק בדברך תזכה בשפטך (תהלים נא:ו) כי אתה הרופא ולא אני	Since, therefore, you have favored me with kindness and have crowned me with honor and glory and you have made me worthy of knowing a bit of the science of medicine, therefore I wish to perform your desire, my elo’ah [And because you should not withhold good from its owner when it is in your power to do it] I am minded^[1] lit, I have spoken with my heart (i.e., mind) to busy myself with the practice of medicine in your Holy Name and through your assistance; “that you may be justified when you speak and be correct when you judge” (Psalms 51:6), for you are the physician, not I.
שאני כחומר בידך יוצר הכל וככלי שבאמצעותו תרפא בריותיך ואני נשען על חכמתי ואיני שם בטחוני על הסמים ועשבים וסגולות שבראת בעולמך כי הם כאמצעיים להשלים רצונך ולהגיד גדולתך והשגחתך	I am but as the clay in the potter’s hand, in the hand of the creator of all things and as the instrument through which you curest your creatures. I do not rely upon my wisdom, nor do I place my trust in the drugs and herbs and medicaments which you have created, for they are but the means to fulfill your will and to proclaim your greatness and your providence.
ולפי שמלאכת הרפואה מסוכנת מאד ועוד כי בער אנכי מאיש ולא בינת אדם לי ומתירא אני פן אמשש בצהרים כאשר ימשש העור באפלה לכן אמרתי להחזיק בשולי רחמיך וללכת אחריך כי באור פניך אהלך ובאורך אראה אור כי אתה נירי ה׳ אֱלֹהַי יַגִּיהַּ חׇשְׁכִּי (תהלים יח:כט)	Since the practice of medicine is fraught with perils, and as I am a man of folly and of no understanding, fearing lest I grope at noonday as the blind grope in the dark, therefore do I cling to the fringes of your kindness and do follow you; in the light of your countenance will I walk and in your light will I behold light, for you are my lamp; “Hashem my elo’ah enlightens my darkness” (Psalms 18:29).
לכן יהי רצון מלפניך ה׳ אלהי ואלהי אבותי שתשכילני שכל טוב מלפניך וחנני מאתך דעה ובינה והשכל והאירה עיני שכלי להבין ולהכיר חלאי הגוף על בורים ואמיתתם מכל פרטי בני אדם שיבואו לידי והורני רפואתם האמיתית לכל אחד בפי מזגו וכפי הזמן הראוי לכל אחד מהם ובעת ראוי לאותה רפואה ולאותו חולה ולא אכשל במעשי ולא בדברי ויראו אויבי וישמחו	Therefore, may it be your will, o Lord my God and God of my ancestors, to endow me with good understanding …and to bestow upon me knowledge and insight and to cause the eyes of my understanding to shine so that I may discern and diagnose the ailment of the body thoroughly and correctly in all the cases that come to me; instruct me as to the drugs suitable to each one in accordance with his disposition and according to the time appropriate to each of them [And when appropriate for that medicine and that patient] so that I may not err in my acts or in my words, lest my enemies see and rejoice.^[2] Cf. Proverbs 24:17)
סעדני ואושעה ממכשול ואל תארע תקלה על ידי	Support me that I do not stumble and that no mishap occur through my hand.
וגומלני חסדים טובים כי לגמול חסד נכנסתי במלאכת הרפואה לפקח על נפשות עמך ישראל	Recompense me with your many kindnesses for to do kindnesses have I entered into the profession of medicine, to save the lives of your people Israel.
לכן סייעני ופדני מחרפה ובזיון אנא אלהי בך בטחתי אל אבושה אל יעלזו אויבי לי ואם יבא לידי איזה חולה שקרבה עת פקודתו ואנושה מכתו יהי רצון שלא אגרום קירוב מיתתו חס ושלום אפילו רגע אחד אלא למדני לתת לו סמים לקיים נשמתו בו עד שבא שעתו ואם ימות יהי רצון מלפמיך שלא יחשדוני אוהביו ורעיו ובני משפחתו ולא יהרהרו אחרי שהייתי סיבת מיתתו אלא יצדיקו עליהם את הדין ויודו שגזרת מלך היא אשר בידו נפש כל חי ורוח כל בשר איש	[Therefore,] assist me and protect me from insult and shame [Please, my elo’ah, in you I trust. Let me not be put to shame. Let not my enemies triumph over me.] If there come to me any patient whose allotted time is about to end and whose affliction is heavy, may it be your will that I cause not the hastening of his death (Heaven forbid!) even by one second. Teach me to administer drugs so as to retain his soul within him until his fated hour arrives. And if he die may it be your will that his friends, his companions and the members of his family do not accuse me and that they do not suspect me of being the cause of his death, but that they accept it as the just decree of God… of the King in whose hand is the life of all living and the spirit of all flesh.
ואם לא קרבה עת פקודתו אלא אתה מדכאו ביסורים במדת רחמנותך כדי שישוב בתשובה שלימה לפניך כי לא תחפוץ במות המת כי אם בשובו מדרכיו (יחזקאל לג:יא) והיה יהי רצון מלפמיך שתגלגל זכות זה על ידי שתורני מה אומר לו להחזירו בתשובה שלימה ושיקבל יסורים באהבה כדי שיהיו כפרה על פשעיו ועל ידי תשלח דברך ותרפאהו ובזה אהיה אהוב למעל ונחמד למטה	But if his appointed time has not arrived and you are casting him down with pains because of your mercy in order that he repent completely before you for you have “no pleasure in the death of the wicked, but that the wicked turn from his way and live” (Ezekiel 33:11), may it then be your will that you cause this merit to fall to my lot and that you instruct me what I shall say in order to bring him to repentance and to accept his affliction with love for you so that it may be an atonement for all his transgressions; and that through me you send forth your command and cure and that thereby I shall be beloved on high, and honored below.
והצילנו משנאה ותחרות ולא אקנא באחרים ולא יהיה קנאת אחרים עלי אלא השכן ביני ובין רופאים אחרים אהבה אחוה שלום ורעות ולא אבוש ולא אכלם בפניהם אלא אהיה מכובד בעיניהם ומאויבי תחכמני וירום ראשי על אויבי סביבותי מחכמתי ודעת שפתי ברור ימללו עד שאהיה לאות ופלא בעיניהם ולא יהא פוצה פה ומצפצף רע עלי ולא על מעשי ואם חס ושלום יאמרו רע לי לא יאמנו דבריהם ולא יכשלו חברי ואשמח בהם ואם חס ושלום חברי ירעו מעשיהם יהי רצון שתשמור לפי מחסום ולא אגלה נבלותם אלא אזכה לתקן את אשר עוותו ותנני לחן ולחסד ולרחמים בעיניך ובעיני כל רואי ושומעי ויעשו מה שצויתי לעשות לחולה אם הרפואה ראויה ואם אינה ראויה אנא ה׳ אלהי הקשה לבם שלא יעשה ושלא יפרסמוה פן אתבזה חס ושלום	Save me from hate and strife. Let me not be envious of others, and may others not envy me. Establish between me and other physicians love, brotherhood, peace and good fellowship. Let me not be put to shame nor disgrace before them, but let me be respected by them. Make me wiser than my enemies. May my head be exalted above my enemies round about me because of my wisdom and the knowledge which my lips may utter clearly, so that I may become an example and model to them and may there be none to utter an evil word against me or my deeds; and if perchance such evil word be spoken let no one give it credence. Let not my colleagues err and me rejoice. If, however, they do evil in their work, may it be your will to place a muzzle to my mouth and let me not reveal their wrongdoing, but may I have the merit to repair what they have injured. Let me find grace and favor and kindness in your eyes and in the eyes of all who see me and hear me so that they do what I order for a patient if the treatment is proper; but if the treatment is not proper then harden their hearts that do not do it; but let them not make it widely known lest (Heaven forbid) I be disgraced.
אנא אל אלהי הרוחות לכל בשר אחת שאלתי מאתך שתחזיק כח זכרוני וכשאלך לבקר איזה חולה הזכירני מיד רפואתו הראויה לו אם למדתיה ואם לאו מתחנן אני לפניך סבת כל הסבות שתסבב סבות וגלגולים או איזה ספר רפואות ואלמד בו רפואתו או איזה ויבוח בין רופאים עד שאזכה לידע רפואתו כי אתה הוא המסבב סבות עולמך כדכתיב והוא מסיבות מתהפך בתחבולותיו לפעלם כל אשר יצום על פני תבל ארצה (איוב לז:יב)	O El, elo’ah of the spirit of all flesh! I ask one favor of you, that you give strength to my memory so that when I go to visit a patient, You make known to me at once which cure will benefit him, whether I have studied it or not. I supplicate you, Cause of all Causes, that you cause the chain of circumstances to act in such manner that you bringest into my hands the medical book in which I may study his treatment or that I may listen to an argument between physicians that will teach me to know his cure; for you are the one who brings about the succession of causes in your Universe, as it is written: “And they are turned around about Their guidance, that they may do whatsoever They command them upon the face of the habitable world.’ (Job 37:12).

אלא בשביל הזכיות שתגלגל על ידי שאפקח נפשות עמך ישראל ועל ידי תצילם מן המות אזכה בעולם הזה לטייל בתענוג הצריך לראות בטובת בחירך לשמוח בשמחת גוייך להתהלל עם נחלתך ועיני תראנה ירושלם נוה שאנן ואתהלך לפני ה׳ בארצוה החיים ואל יזדקק שטן במעשי ידי כי לזכות ולטהר באתי׃	But for the merit which you may bring to me in that I may preserve lives of Israel, your people, whom you will save from death through me, grant that I may deserve to walk about this world in the pleasure of beholding the good of your chosen ones, rejoicing in the joy of your people, of glorying in your inheritance. May my eyes see Jerusalem, the peaceful habitation. May I walk before Hashem in the land of the living. Let not Satan attach himself to the work of my hand, for to purify and to cleanse have I come!
אלהי פלטני מיד רשע מכף מעול וחומץ ואל תמסרני אפילו רגע אחד בידו פן ישיאני להתעולל עלילות ברשע חס ושלום לתת איזו רפואה ממשקה או סם להזיק שום אדם או להפיל אשה הרה חס ושלום ואם ישיאני קדמה פניו הכריעהו פלטה נפשי ממנו ה׳ עשקה לי ערבני כי אתה תקותי ה׳ אלהים (ישעיה לח:יד) מבטחי מנעורי	My elo’ah, deliver me from the hand of the wicked, from the palm of the pervertor and oppressor and place me not in his hand even for one moment lest he entice me to practice wantonness (Heaven forbid!) to administer a poison or drug to injure some man or some pregnant woman. (Heaven forbid!) If he try to entice me, meet him, humiliate him, deliver my soul from him. “O Hashem, I am oppressed, be you my surety” (Isaiah 38:14), for you are my hope, o Hashem Elohim, my trust since my youth.
וטהר מחשבותי וזכה שרעפי לבל אחשוב מחשבה רעה חס ושלום על שום אשה בין בתולה בין בעולה בלכתי לבקרה ולא אתור אחרי לבבי ואחרי עיני (במדבר טו:לט)	Cleanse my mind and purify my thoughts that I think no evil about any woman, whether virgin or wife, when I visit her that I “do not go about after my own heart and my own eyes” (Numbers 15:39).
והצילני מכל היזק וחולי ומדוה המתרגשות ובאות בשביל החולים יומם השמש לא יכני וירח בלילה (תהלים קכא:ו) אם אצטרך לבקרה בלילה	Save me from all injury, disease, infirmity, [and pain of the afflicted] “May the sun not smite me by day nor the moon by night” (Psalms 121:6) [if I must visit her at night.]
ואנא בעל החסד והרחמים גל עיני ואביטה סודות מעשיך הנפלאים ואדע סגולות הרפואה אשר יסדת בעשבים ובאבנים בזרעים ובפרחים בשרשים ובעלים בעצים ובפירות ביינות ובשמנים במימות ובשאר משקים בבעלי חיים אשר בשמים ממעל ואשר במים מתחת לארץ בפשוטים ובמורכבים ובהם אגיד זרועך לדור לכל יבא גבורתך	I pray, o you Master of kindness and mercy, open my eyes that I may discover the secrets of your wonderful deeds and that I may know the peculiar curative powers which you have placed in herbs and minerals, in seeds and flowers, in roots and leaves, in wood and fruit, in wines and oils, in waters and in other liquids, in living organisms which are in the heavens above and in the waters under the earth, in simple and in composite structures and that through them I shall tell of your might to all generations to whom your greatness shall come.
יהי רצון מלפניך שתשלח ברכה בכל מעשה ידי ובשכר שיתנו לי על טרחי ואראה ממנו סימן ברכה ואזכה להשתמש ממנו על מצוה טובה וישרים לפניך ולהגדיל תורה ולהאדיר	May it be your will to give your blessing to all the works of my hand and to recompense which is given me against my will so that I may look upon it as an omen of good and that I may apply it to good and righteous purposes before you, and to magnify and glorify your Instruction.^[3] Torah, “instruction” or “teaching.”
ויהיו מזונותי ומזונות זרעי וזרע זרעי מסורים בידך ולא ביד בשר ודם ויהיו בריוח גדול כדי שלא אצטרך ליטול דבר מן העניים חולים אלא אדרבה אזכה לחננם ממה שהננתני כי ממך הכל ומידך אתן לך	And may my sustenance and that of my children and my children’s children come from your hands and not from the hands of men of flesh and blood, and may it be in abundance so that I be not forced to take anything from the poor and the sick but, on the contrary, that I may be able to give unto them what you have bestowed upon me, for from you come all things and from your hands I return to you.
ותזבדני זבד טוב מלפניך שבכל אשר אעשה אשכיל ואצליח ובפרט במלאכה זו תגדיל עמי חסדיך שיקראוני לרפואת חולים אשר אפשר להם הרפואה ויהו נרפאים על ידי עד חיותם	Endow me well that I succeed and prosper in all that I do, and especially in the work of healing do you magnify your kindness so that patients call me to whom cure is possible and who may be cured completely through me.
ואשר הם בלתי אפשרי הרפוא ואנושה מכתם שגזרת שלא ירפאו מאותו חולי עד יום מותם אל יקראוני לרפואתם בסודם אל תבא נפשי בקהלם אל תחד כבודי (בראשית מט:ו)	And let those not call me who are incurable and whose illness is fatal because you have decreed that they may not be cured. “Let my soul not come unto their council; unto their assembly let my glory not be united” (Genesis 49:6).
ואם יכבדוני בני אדם בשביל חכמתם אל תבואני רגל גאוה אלא נפשי כגמול עלי אמו כגמול עלי נפשי (תהלים קלא:ב)	If people consult me because of my knowledge let not the foot of pride overtake me, but let my soul be “like a weaned child with his mother” (Psalms 131:2).
ואל ישלוט בי יצר הרע ועין הרע ולא יחשדוני בשום דבר בעולם ואל תביאני לא לידי נסיון ולא לידי ביזיון והצילני מהרהור חטא ועברה ועון מעתה ועד עולם והלבש את נפשי בגדי פאר וכלי שרת הלא הם עטרת תפארת של מדות טובות הראויות למוצאים חן בעיניך	May no evil desire nor evil eye have power over me. [And may I not be suspected of anything in the world.] Let me not be brought into temptation or shame. Save me from the reflection of sin, and transgression and iniquity, now and forever, and clothe my soul with glorious raiment [and tools of practice], the crown of glory of good morals, as is becoming to one who finds grace in your eyes.
וחזק כח הרגשת כל חושי שיגידו לי האמת מכל מה שאעביר לפניהם ולא אטעה בשום אחד מהם לא בטעם ולא בריח לא בראות ולא בשמע ולא במישוש אלא יאמר לי על הין הין על לאו לאו על מר מר על מתוק מתוק כדי שלא יגיע נזק לחולה בשביל חלשת הרגשם	Lend strength to all my senses so that they may tell the truth in everything which is brought before them, that I may not be mistaken in any one of them, neither in taste, smell, sight, hearing nor touch, let them tell me concerning what is positive that it is positive, what is negative, negative, what is bitter, bitter, of what is sweet, sweet, so that no injury result to any of my patients because of the weakness of my perceptions.
וכן חזק כח הדבור של חולה וכח שכלו וזכרונו שיגיד לי האמת מסבות חליו ולא יטעה בהם כלל ולא ישקר לי בהן לאיזה סבה פן חס ושלום אשוה לו גם אני בטעות אלא אזכה להבין סבות חוליו מתוך דבריו ומתוך סימני חליו	Strengthen likewise the power of speech of the patient and the vigor of his understanding and his memory so that he tell me the truth as to the causes of his ailment and no falsehood in regard to them lest I, also, be led into error [Heaven forbid!] and thus fail to understand the causes of his disease from his words and from the symptoms of his disease.
ואהי ראוי וחכם להגיד לחולה העתידות לבוא לו כדי לאמת דברי ואזהרותי אליו וישמור כל אשר צויתיו סמכני באמרתך ואחיה ואל תבישני משברי (תהלים קיט:קטז)	Let me be fit and learn to prognosticate correctly to the patient in order that my words and warnings may be verified and my injunctions observed. “Uphold me according unto your word, that I may live; and put me not to shame in my hope” (Psalms 119:116).
ואל תפח נא מלבי את תקותי אל תפן לרשעי ואל תתעלם מבקשתי וחנני וענני ושמע תפלתי כי אתה שומע תפלתי ועל הכל אני חייב להודות להלל ולשבח לשמך כי טוב נגד חסידיך תרב גדולתי ותסוב תנחמני ויהיו דברי אלה אשר התחננתי לפני ה׳ וגו׳.	Do not destroy hope from out my heart; do not incline me to do evil; do not hide yourself from my supplications; be gracious unto me and answer me; hearken unto my prayer. Indeed you hearkenest unto my prayer and I must give thanks for everything and sing halleluyah and praise your name, for goodness is in store for your kind ones! May I increase in greatness and may you comfort me! May these words which I have supplicated to Hashem come to pass, etc.

This is the Physician’s Prayer of Yaaqov ben Yitsḥaq Tsahalon (1630-1693) published in his ספר מרגליות טובות Sefer Margaliyot Tovot (Venice, 1665). The Hebrew text is here set for the first time side-by-side with an English translation largely derived from that made by Harry Friedenwald (1870-1944) as published in his article, “Jacob Zahalon of Rome: Medieval Rabbi, Physician, Author and Moralist” (Bulletin of the Medical Library Association. Jul;8(1):1-10.1918). Archaisms have been replaced, divine names re-Hebraized and gender neutralized, and a word replaced here and there with one that seems more suitable to me. A handful of lines and phrases left untranslated by Friedenwald are here translated and included in brackets.

The transcription follows JTSA Ms. 4326 used for the source of Friedenwald’s own unpublished transcription. All ambiguities were checked against the handwritten manuscript copy of Sefer Margaliyot Tovot. –Aharon Varady

Allgütiger! Du hast des Menschen Leib voller Weisheit gebildet. Almighty El, you have created the human body with infinite wisdom. Ten thousand times ten thousand organs have you combined in it that act unceasingly and harmoniously to preserve the whole in all its beauty the body which is the envelope of the immortal soul. They are ever acting in perfect order, agreement and accord. Yet, when the frailty of matter or the unbridling of passions deranges this order or interrupts this accord, then forces clash and the body crumbles into the primal dust from which it came. This prayer was written by Markus Herz (1747-1803), a German-Jewish physician and lecturer on philosophy. He was a pupil of Immanuel Kant and physician to Moses Mendelssohn. His prayer was first published in 1783 as “Tägliches Gehet eines Arztes bevor er feine Kranke besucht” in Deutches Museum (1, pp. 43-45). A Hebrew translation by the editor of the periodical Ha-Me’asef, Isaac Abraham Euchel (Itziq Eikhel, 1756-1804), followed in 1790 (6, pp. 242-244). For a history of the misattribution of this prayer, see Fred Rosner’s article, “The Physician’s Prayer Attributed to Moses Maimonides” in the Bulletin of the History of Medicine (41:5, Sep/Oct 1968, pp. 440-454.).

After this publication at the Open Siddur Project, Chana Deutsch translated the prayer from its original German into Magyar (Hungarian). The first English translation was published in the London periodical Kol Yaakov in 1841, however, I was not able to locate a copy. The English translation appearing here was made by Dr. Harry Friedenwald as published in “The ethics of the practice of medicine from the Jewish Point of View,” in the Bulletin of the Johns Hopkins Hospital (28, 1917, pp. 260-261). I have replaced archaisms and re-hebraicized divine names where they appeared in his translation. –Aharon Varady

I wrote this kavvanah [around 2010]. At that time I lived in Ithaca, New York. I was a substitute teacher in the Ithaca Central School District. There was a community event at Fall Creek Elementary school, and the way families, faculty, students, and people from the area came together inspired the poem.
