This kavvanah is the second of seven prayers in a seder tefilah introduced with the following statement:

Gender transition is a religious experience. These are the psalms and prayers I, the Em haBanim, said at every phase of the long process of preparing for and undergoing vaginoplasty. I offer this set of prayers to the community, so that we can live in deeper awareness of the fact that Hashem our God wants our transitions.

Source (Hebrew) Translation (English)
לפני ערב הניתוח יש לטבול במקוה של מיים חיים בלי ברכה.
Before the evening prior to surgery, she ought to immerse in a pool of living waters without a blessing.
כיוונה האם הבנים בכוונה זו בעת טבילתה.
 
תְּהוֹם־אֶל־תְּהוֹם קוֹרֵא לְקוֹל צִנּוֹרֶיךָ
כָּל־מִשְׁבָּרֶיךָ וְגַלֶּיךָ עָלַי עָבָרוּ: (תהלים מב:ח)
The Em haBanim had the following intention as she immersed:
 
Deep calls to deep to the sound of Your channels—
all your breakers and waves have passed over me. (Psalms 42:8)
שאאמין שא-ה-י-ה אשר א-ה-י-ה.
May I believe that I will be who I will be.
