מִי שֶׁבֵּירַךְ אֲבוֹתֵֽינוּ
אַבְרָהָם יִצְחָק וְיַעֲקֹב
מֹשֶׁה וְאַהֲרֹן
וְדָוִד וּשְׁלֹמֹה׃
הוּא יְבָרֵךְ אֶת כׇּל־הַקְּהִלּוֹת הַקְּדוֹשׁוֹת וְהַטְּהוֹרוֹת,
גְּדוֹלִים וּקְטַנִּים
הֵם וּבְנֵיהֶם
וּנְשֵׁיהֶם וְתַלְמִידֵיהֶם
וְכׇל־אֲשֶׁר לָהֶם׃
May the One who blessed our ancestors,
Avraham, Yitsḥaq, and Yaaqov,
Mosheh and Aharon,
David and Shlomo;
bless all the holy and pure congregations,
both great and small;
they, their children,
wives, disciples,
and all belonging to them.
מַלְכָּא דְעָֽלְמָא הוּא יְבָרֵךְ יַתְהוֹן
וִיזַכֶּה יַתְהוֹן
וְיִשְּׁמַע בְּקָל צְלֽוֹתְהוֹן
וִיקַיֵּם בָּהֶם מִקְרָא שֶׁכָּתוּב
יְהוָה אֱלֹהֵי אֲבוֹתֵכֶם
יֹסֵף עֲלֵיכֶם כָּכֶם אֶלֶף פְּעָמִים
וִיבָרֵךְ אֶתְכֶם כַּאֲשֶׁר דִּבֶּר לָכֶם (דברים א:יא)
(בשרת ימי תשובה אומרים:
וְיִכְתְּבֵם בְּסֵפֶר חַיִּים טוֹבִים)
וְכֵן יְהִי רָצוֹן
וְנֹאמַר אָמֵן׃
May the supreme King of the universe, bless them
and purify them;
and hearken to the voice of their supplication:
so that he may fulfil in them, what is written,
[that] “YHVH, eloah of your ancestors
make you a thousand times as many more as you are,
and bless you as he has promised you.” (Deuteronomy 1:11)
(During the Penitential Days, say also:
And may Hashem write them in the book of life.)
Which in Hashem’s infinite mercy grant:
and let us say, amen.

This is the mi sheberakh prayer, the Brakhah la’Qehilot, (Blessing for All the Jewish Congregations [worldwide]) as found in Seder haTefilot: The Order of the Daily Prayers in Hebrew and English According to the Custom of the Spanish and Portuguese Jews (1789, 2nd ed. 1810) p. 130v-131r. I have re-hebraicized names, replaced archaisms, and made other minor edits to the translation. –Aharon Varady
